Intake Forms

Once we receive your information, we will send you a link to the appropriate new client paperwork via our secure patient portal.

• For Individual Adult Therapy: Please complete the paperwork and submit it through the patient portal.

• For Individual Child/Adolescent Therapy: Please have the parent/guardian complete the paperwork and submit it through the patient portal.

• For Couples Therapy: Please have both partners complete their respective paperwork and submit them through the patient portal.

• For Family Therapy: Please have one or both parents/guardians complete the family paperwork and submit it through the patient portal.

• For Coaching: Please complete the coaching intake documents and submit them via email to Tamara@BeyondNowCo.com.
